Listing absences on ILR application

My original passport did not have a surname as the surname was missing from my school certificate, which is considered as a legal document for name in India. A few years ago (before I came to UK) I got a notary to declare that I have changed by name and the surname was added. My latest passport has the surname in there.
The ILR application requires me to declare the previous names. Do I need to declare the previous name without the surname? If so, how do I do that, as surname is a mandatory field?

You could double ur name- since the field is mandatory. Or write xyz. Write a cover letter and explain the situation that you had to write it coz the form has technical issues.

Never heard that as usually the Birth certificate/national id card is considered legal document.

The ILR application has a question - Which countries did you visit or travel through when absent from the UK and Crown Dependencies?
The dropdown allows only one country at a time. If I visited 2 countries in the same trip, do I need to repeat the section for each country with the same set of dates for leaving and arriving in UK?

Add the final destination. They are more interested in the date you left UK and came back to UK. Add the transit destination in the “reason” section and mention those dates there.
