You need to provide much for information on exactly what happened in this period from refusal and spouse ltr.

Was your appeal successful, did you withdraw the appeal? What do you mean by "ongoing"? Did you apply for a spouse visa while an appeal was pending?

21/9/2015 Judge decision to reconsider my case

Note: after withdrew of appeal i have applied spousr visa within 14 working days and granted on first attempt.

Withdrawing your appeal broke your section 3C protection and made you an overstayer from 27/6/2016.

What is it saying in comments/notes section about FLRM; out of time/ in timely. There usually be a comment on the SAR report about each historic application pacifically distinguishing whether these were out of time or in time.

Yes there is written after my first spouse granted details it shows:

Are the requirment at paragraphs E-LTRPT 2.1 and 2.2 met ? YES

Does the applicant have extant leave? Yes (Because his appeal is not decided) if NO, are there any exceptional reason why hey could not apply in time and within 14 days of the expiry? N/A

Set lr has no concept of including any dependant and their documents.
