Tier 2 (General) Dependant to ILR

Hi,
I was on Tier 2 (General) visa and shifted to ILR in January 2017.
I completed British citizenship in Nov 2018
My wife joined me on Tier 2 (General) dependant visa in 2015
She extended her visa on 2018
Now she is due ILR
Which form shall we use?
SET O or SET M

Set O is the only form she can use I'd she has held and still holds a PBS dependent visa. This is provided you got ilr based on 5 years residence and not 10 years long residence.

Yes I got it based on 5 years being on Tier 2 (General)
She is still on PBS Dependant visa

Regarding this question:
What VAF number should be given ? One I had on my Tier 2 General Visa or the one on my ILR visa.
As currently I am already on British passport already.
What would be GWF number?
