Documents required for ILR 10 Years Long Residence

Hello all,
Sorry I am new here. My student visa started on 01/04/2011. So, I am trying to apply for my ILR today. [28days before my qualification period]
I have 2 UK degree certificates, passports for the past 10 years and have already passed my Life in the UK test.
Do I need to prepare bank statements or anything else?

Not required for ILR based on 10 year route

Dear CR001,
I have been in the UK for 14.5 years. But I had a gap before 01/04/2011. At the time, the college was trying to apply for a licence for a student visa over 18+ so I was in the UK while waiting for my new visa. The situation at the time was tricky and we had a lawyer and everything was fine after. I have applied 10 years ILR back in 2016 but was rejected because of the 3 months gap.
I have attached an image of my Immigration history back from my previous application back in 2016.

Sorry to bother you guys. I have an appointment in 2 weeks. I just want to make sure I upload all of the required documents in the right categories.
Proof of Application:
UK Access form
Consent Form
Educational:
Only UK Degree certificate
Proof of identity/Travel History:
BRP Card
Current Passport/Previous Passport [All pages]
ID card
Other:
Police Registration
Bring QR code and all of the doc on the date.
I am not sure Which Category I have to upload my Consent Form to. Thank you for your help and Looking forward for your reply.
