PBS dependent (ILR dependent) visa fees

Hi all,
Iam currently on a health care worker visa and will be eligible to apply for ILR in few months . My dependents arrived the UK 16 months after me so they will not be eligible for ILR at the same time as me. I have 2 questions:
1- Either for me as a health care worker or my dependents we used to pay low fees £247 PP for visa extension with NO IHS fees. so after i get my ILR , if i need to apply for my dependents for PBS dependent visa , will i will pay for them same fees i used to do as health care worker dependent and no IHS or will be a different story ?
2- If i want to delay my ILR application and ask my employer to issue COS for health care worker visa extension again which i do on yearly basis, will the home office limit visa duration to end by the time i will be in the UK for 6 years ( as this is as long as you can stay on your visa without applying for ILR) or they will issue the visa based on employer’s request in the COS ?

If you got ILR through your visa route (not through long residence), they should be able to apply as health and care worker dependent even after you got ILR, since that's part of the SW route.

SW 36.1. A partner will be granted:
(a) permission which ends on the same date as their partner’s permission as a Skilled Worker; or
(b) 3 years’ permission if the Skilled Worker was (or is being) granted settlement as a Skilled Worker.

If you want to stay longer in the UK
You can apply to extend your visa as many times as you like as long as you still meet the eligibility requirements.

Probably this was a misunderstanding from me that i have to apply for ILR after 5/6 years which is the SW for my visa .
Though, iam still not sure about the fees for dependent application after i get the ILR as not all skilled worker dependents fees are the same and only the health care worker are exempted from IHS if iam right , unless during my application for them , i will be asked what is my current post !

take caution as their situation is different, person was on ILR before their dependents arrived it seems
that is indeed unclear!
maybe you can ask your sponsor to direct the query to UKVI?
