ILR Form for 12 yr old child

I had my child as an unwed single parent and no father. His birth certificate has only my name as the mother. No father showed up till I got married 9 years ago. My husband has filled in as his father since we married. I can also prove sole responsibility
I moved to the UK with her 5yrs ago and we are applying for ILR now. My question is this:
I am confused with these two questions below in the SET M form..
1. Child's parents or legal guardians
Confirm who your parents or legal guardians are
(Required)
Parents include biological, adoptive and step-parents. You can select more than one option. If you need to add multiple people not on this list, you will be able to do that later on.
My name and my husband names are given as options. Do I select my husbands name as well since he is a step father or only my name?
2. your partner's relationship to your CHild
What is your partner's relationship to your child?
(Required)
a. Parent
b. Step-parent
c. Family member
d. Legal guardian
e. Other
What option should I select above? Step-parent or Family member?
