New form: How to select Tier1 and Work Permit combination?

Hello,
Can any of senior members look into the new form

How can a persion who was in Work permit and later Tier1 apply for ILR?

How can the combination option be selected?

Guidance document is of not much help it seems :(
The qualifying periods are as follows:
5 years In the work permit holder, employment not requiring a work permit, businessperson, innovator, investor, self-employed lawyer, writer, composer or artist, UK ancestry, Tier 1 and Tier 2 categories or routes.

Please tick a box below to show us the category in which you are applying for indefinite leave to remain.
Note the use of singular tense (category in which) and the present tense (you are applying).

The form doesn't ask for "combination" of categories, but your current category - on the basis of which you are applying for settlement.
