Hi all,

In SET(O) Section 1 - Applicant's Details, question 1.4 says: "Your full name as in your passport or travel document" and 1.5 says: "Surname or family name"

If my full name in passport is e.g. John Johnson, where my first name is John and surname is Johnson, do I need to answer 1.4 as John Johnson or just John?

Obviously, 1.5 would be Johnson.

How is it in your passport

Is it Surname : Johnson

First Name : John

If that is the case, as it is in my case, You give John for 1.4 & Johnson for 1.5
That is what I did.

(My Name is Sreekanth Muraleedharan Nair) In Passport it is

Surname/Family Name : Nair
Name : Sreekanth Muraleedharan

So I Entered Sreekanth Muraleedharan in 1.4 & Nair in 1.5.

This question is a bit ambiguous given they're asking for full name and not just name, which I found confusing.

seems full name is name+surname, although they're asking for the surname separately in the next question.

Furthermore, the same page in SET(O) form contains guidance related to photographs which states that one should write a 'full name on the back of each photograph'. This definitely means name+surname.

Seems that this would be correct:

1.4: John Johnson

6.1 When did you (the main applicant) first enter the UK? This refers to the
date of your first entry into the UK at the beginning of the period of stay
on which this application is based.

I got UK Visa on 16/01/2007 but entered UK on 27/01/2007. Which date shall I put?

6.3 When did you last enter the UK?

The last time I left UK was in August, went for a holiday and came back on 29/08/2011. Is this the date they're asking for?

The date is you entered the Uk is the 27th as against when your visa was issued.

Yes this is the date that is been requested of.

Hi, I am also confused about section 1.4 and 1.5 of form SET(O).

1.4 = ? (full name= name + Surname ????????????) please senior members and professionals please throw some light on this please.

What can I fill in 1.4 ? my first name only or my complete name (name+surname) ????????????//

My Visa is stamped in my old passport. What shall I answer to question

6.7 - What evidence of leave (or permission to stay) stated in 6.6 has been provided?

1) Passport
2) Travel Document
3) Biometric Residence Permit
4) Other
5) None

Shall it be 1) Passport or 4) Other - as it is a visa in the old passport?
