I have lot of questions regarding out of UK application after looking online application.
So I am margin all my Questions in one post.
Hi My wife applying for SOC 2112 Biological scientists and biochemists her salary is 21,600£.
Is following point claimed is right in online application?
Points Claimed

Certificate of Sponsorship (shortage occupation) 0 Do i have to write 0 or N/A or leave it blank ?

2.Is visiting GP does counts as medical treatment? Because we visit many times to GP
Have you made an application to the Home Office to remain in the UK in the last 10 years?
Have you ever travelled to the UK in the last 10 years?
3.For me it is over 10 year so do I need to give application/Travel details over 10 years
4.If we both apply together how do I book appointment online for both of us?
5.If I want to apply as dependent of my wife within UK what are the ways to apply? I cannot find tier2 dependent visa online. And paper based form is very old

You answer yes, you are entitled to use the NHS.

Have you made an application to the Home Office to remain in the UK in the last 10 years?
Have you ever travelled to the UK in the last 10 years?

You to list what the form is asking for. Have you read the Set(O) guidance notes?

4.If we both apply together how do I book appointment online for both of us?

Assuming you are applying for a Tier 2 General extension, book an appointment for main +1

5.If I want to apply as dependent of my wife within UK what are the ways to apply? I cannot find tier2 dependent visa online. And paper based form is very old

You answer yes, you are entitled to use the NHS. In past 10 year i was register lot of GP and i dont remember all my visits, here if i say yes it ask all address and visit to GP. I never received treatment, i visited GP only

You to list what the form is asking for. Have you read the Set(O) guidance notes? If you wish to apply for indefinite leave to remain in the UK on form SET(O), please read these
guidance notes. I am not applying for ILR, i am applying for Tier2 G

Assuming you are applying for a Tier 2 General extension, book an appointment for main +1 It is not extension application, it is new Tier2 G application out side UK

Balance:
We have monthly bank statement but when we apply for visa we will not have recent bank balance statement.
So in safe side i will add following documents
Monthly bank statement +
Online bank transaction print with bank stamping and signature (which show only sort code and account number, no other details)+
Mini-Statement print from bank (Which show only name and sort code and account number and lot of stander bank details).
Does any one used Mini-statement from santander bank to show recent transaction for maintenance fund as evidence with monthly bank statement?

Offer letter:
At what stage employer have to issue offer letter.
Before CoS request OR after CoS approval on 11th ?
