2015 P60 different employer, accountant letter

Dear all,

I am newbie here but I’m seeing everybody really helpful and I’m sure you’ll give me some ideas about my situation as well. Next week I’ll apply settlement under representative of an overseas business visa. But my first P60 in 2015 (just for two months) is under formal representative name. I have got letter from the accountant why she paid two months under this scheme because Hmrc opened the new one late. So what should I do?

1-should I write cover letter and attach accountant letter? Or
2-not mention anything and just scan and put it on system as it is.
