Which application should I use for ILR ?

I need to apply for ILR for my partner.
Could you plz advise which application should I use for my partner ILR? I am confused between Set(M) and Set (O)? Plz only answer if you are 100% sure.

She entered the UK as ILR dependent: Spouse/Partner LEAVE TO ENTER (Type of Permit).
Extended as a British citizen dependent: Souse/Partner LEAVE TO REMAIN (Type of Permit).
She is completed 5 years in the UK and eligible for ILR.

I earned ILR by completing 5 years on Tier 1 (General) of the Points Based System then British citizen.

Plz advise.

If she applied for and came to the UK as your dependent AFTER you got ilr, then it is form SET M.
