My questions:
1) Does my wife and/or child need to sign the PART 1?
The checklist only have asked me to sign it. Considering they are also the applicant, so I am not sure if it's needed.
You sign Part 1 and your wife part 2. If you have someone else who support you financially ask them to sign Part 3.
2) Do I need to sign the PART 2: Consent for the Home Office to verify third party information?
I provided the bank statement with only my name on it. For the evidence of cohabitation, I supplied some letters addressed to me and my wife. So does my wife need to sign PART 2?
No, you have to sign only Part 1. Applicant is yourself and your wife and daughter are your dependants.

I reviewed my documents, and found that I made 2 mistakes:
1. I reported the change of address to the police late for 1.5 year, I simply have forgotten this...
2. I renewed the passport, but didn't realize that this also need to be reported to the police. It's 0.5 year late...
Should I mention this in the cover letter and say anything? Will the mistakes cause my application been refused?![]()
