I am applying for Tier 1 HSMP and also applying my spouse dependant application along with my main application

Question:

In section B11
B11. Give details of any current or previous worker reference number(s) under work permit arrangements (if applicable):

Should we list the dependant workpermit numbers

I am applying dependant and main application together so for below question what option should i select

H3. Under which category of the Points Based System has the main applicant applied?

As I am applying HSMP for the first time will i fall under any HSMP Forum judical review policy?

i have this below question in both main and dependant applications:

"Has the main applicant applied to have his/her application assessed in accordance with paragraphs 39-42 of the HSMP Forum Ltd. Judicial Review Policy Document?"

Should we select YES or NO for above question.

Any help would be highly appreciated.

1. You you have previous work permit reference in Uk, then mention it otherwise it is not applicable to you both.
2. You said this is your first time so you don't need to mention it.
