Help in form filling for HSMP (tier 1)

Hi,
I need help on following :

PBD Dependent form for hsmp extension Tier 1
"Section 2 D9:
Give details of journey including where the dependant travelled from ticket booking or reference number and transport operator"

i have journey details and transport operator but i dont have ticket booking or reference number it was e-ticket and it was the printout in May 2008. I dont have that details what shall i do

please advice

Hi,

Sorry guys just found that D9 is for the applicants who travelled and which shows his / her current leave unavaiable

so i am fine as i dont need to fill the booking reference number
