1. visa application form

2. recent passport-style photo
3. passport or travel document valid for at least three months after visa expiry date
4. copy of the EU or European Economic Area citizen’s identity document or passport along with a declaration requesting the presence of the family member in Italy and swearing to be in possession of the requisites foreseen by the law.
5. administrative documentation (marriage certificate, birth certificate, etc.) attesting to the familial relationship
6. in the case of minor children, written permission from the child's other parent for visa to be issued

Entry visas?
Member States shall grant your third country family members every facility to obtain the necessary visas. Such visas shall be issued free of charge as soon as possible and on the basis of an accelerated procedure. Member States may not require family or residence visas for your family members but only entry visas.
Documents that can be required when applying for the visa !
The right of entry of your third country family members is derived from their family ties with you, a Union citizen. All the Member State consular officials can ask for is their passport and a document establishing their family ties with you, such as marriage or birth certificate and proof of dependence, where applicable. Your family members cannot be asked to present documents such as travel tickets, employment certificate, pay slips, bank statements, proof of accommodation and means of subsistence or a medical certificate.

As your right of entry is derived from your family ties with a Union citizen, all the Member State consular officials can ask you to produce for the visa application is your passport and a document establishing the family ties with a Union citizen, such as marriage or birth certificate and proof of dependence, where applicable.
Consular officers issuing the visas may not:
