SPOUSE VISA INFOS NEED?

HI THERE,
I would like to know, under which category name comes the Spouse visa, when it comes to download the application form from the UK Border agency?
I am confused as I do not see Spouse visa, but EEA FAMILY PERMIT,OR VAF1F, which one is the spouse visa please?
Also, when applying for this visa do my future partner need my birth certificate?

For applications outside the UK, Spouse of British or settled/PR person may apply for a spouse visas, subject to maintenance and accommodation requirements.

Spouses of EEA nationals may apply for an EEA family permit.
