Can I provide all the three documents
a) Bank statement
b) Payslip as mentioned above
c) a letter from employer with monthly breakup of gross and net to bank.

My payslip shows two gross figure - Salary and Gross salary and third one net. the gross salary include allowances (Car). Which one should I mention , salary with Car allowance or without car allowance ?

as I have not yet received my last months bank statement and it is expected to take some time, Is it OK to apply with last month bank statement taken from bank branch on letter head, signed and stamped ?

Main Application:
1) While applying my initial application 3 years back for tier 1, the application was clearly mentioned to put 'X' for selection but in this new application it is mentioned as 'tick' - Should I use 'X' or tick mark here?

2) One of my recent payslip was lost in post and therefore I have got the
payslip printed on company letterhead, sealed (Payroll dept) and signed by authorised person for that month - Would this payslip be considered?
As said earlier, YES, its valid
3) Section B14 - Point Based System migrant reference number (if known) - Do they mean the PBS reference number at the time of initial application (3 years back)?

4) Section D - I was initially started in 2006 with work permit visa converted to tier 1 (General) Nov 2008 and was out of country from end Jan 2009 to Early May 2009 but still had Tier 1 Visa. In Immigration section D2, D3, D4 do they mean the first entry clearance visa ref in 2006 or the recent one Tier 1 (where I was back in May 09).

5) Section H4 - Do I need to send my original degree for extension when there is provision to select 'evidence previously provided with initial tier 1 application.
Not sure. Better to send it.
6) How recent (at the time of application) the 3 months bank statement should be for the proof of maintenance fund.
Should not be older than one month
Dependent Application:
1) My Family are in UK for more than 12 months but less than 2 years - Should the maintenance fund be £533 each or £1600
That depends on how long YOU are in UK. Not ur family
2) Section K4 - Maintenance certificate by sponsor - this section is not clear to me, who is A-rated sponsor here? Proof of maintenance fund is provided through bank statement but do I need to additional declare in section K4? Not sure who is A-rated sponsor here.
Even m not sure about this bloody question. Ideally i think it should be left as it is. (if you are spponsoring ur family)
3) Section 5 - If the maintenance fund is in the name of main applicant then who would be signing section 5 for consent for verification to the bank - Main applicant or dependent?
In this case, Page 36 top dependant should sign, Page 36 bottom to be left blank, and Page 37, main applicant's details
4) Does my spouse need to show proof of residence with to separate documents ?
Marriage certificate is MANDATORY to show again. Here, I assume you are going to put both the application in to same packet while applying.
