damaged non-EU passport with UK visa inside - what to do?

Hi

I am UK citizen living in Germany.

My wife is from Ukraine and travels under a Ukrainian passport.

Inside that passport is a 5 year UK visa, issued earlier in 2011.

That passport is now damaged - the photograph has fallen out while in the UK for a family visit. The German border guys let her back into Germany, after all she also had her (German) residence permit, but of course told her she needs a new passport.

If she gets a new passport, is the visa in the old one still valid?

If not, what is the procedure to get that visa transferred to a new passport?

Is there anyone/anywhere we can/should write to to get a definitive, official response?

Am I reading this right? It strongly suggests we need do nothing except retain the old passport along with the new one?

That's probably assuming that the old passport was not damaged, with the photograph falling out.
