Some query about Set(LR), please someone help me. Urgent!

Hi there! I am gonna send my Set(LR) application tomorrow, can anyone please help me to check my documents listed below to see if i need anything else to add into my application, Thanks!

I am applying using 10 years Route. so i suppose the correct form shall be Set(LR)?

below are the documents i prepared for this application:

1. Set(LR) form

2. 2 Passport Photos both with my name written at the back of it.

3. 2 Passports (old and current ones)

4. Police Registration cert

5. Life in the UK cert

6. Bath University Beng degree cert (only qualification , not transcript)

7. several personal references from UK citizen to support my application.

Are these documents sufficient already? Do i need to send my transcript along with my degree cert to meet my English qualification requirement?

About the Photo, i took my photo from a photo booth called 'photo me' in Boots as a set of 'passport photos', but the photo booth also provides a option of 'visa photos' set, which set shall i use?

I also have some query about the application form

--1. on page 4, Set(LR), Section 1, Question A6 it asks me to choose the method of payment.
so i chose Postal Order, and it asks me to go to question A19, but i cant find A19 anywhere in this form. The last one of Section 1 is A18. What can i do?

--2. On page 7, Section 2, Question B15 asks to write the date i started living at my current address.
i actually moved into my current house about 1 year ago, but the day i told police that i moved was 10 month later. So what date shall i write here?and how does this kind of late registering thing affect my application?

--3. Still the address issue. During last 10 years i sometimes forgot to register my new address in police station after moving home, this happaned about 2-3 times. Should i write down those addresses if the application asks me to list ALL my previous addresses? And for the starting date of my previous address, should i use the same date as the registration date on the police reg cert or the date i actually moved in?

--4. still page 7, question B15, regarding Previous address.
How many addresses should i put there? All my previous addresses ? The application form provides a few spaces to list only one of my previous addresses, if its more than one, then i dont know where else i can write down the information.

One last question, I once got rejected 10 years ago, how does that affect my ILR application then? That case was like this: 31 days before my visa expired, i went in person to renew my visa, i got rejected due to lack of documents. then after 29 days i went to Croydon again and this time i got my visa approved.

Please some experienced members give some advices for me, i would be grateful if anyone here can help! Thank you!
