Application to extend Tier 2 dependent visa

Hello,

I have been granted an ILR in August this year (Tier 2 (general) 5 year route). My wife did not meet the continous stay requirement so she could not apply for ILR with me. Her Tier 2 dependent visa is going to expire in December and we would like to apply for an extension. Can anyone please direct me to the webpage for this specific application form?!

Note, for the IHS, you put your ilr expiry as the date you apply and year 2022 to get IHS of £1200 for a 3 year extension. Do Not put your exact expiry of 2024.
