T1 Entre. Dependent ILR eligibility

Hello all,
I am new to this forum. I came in UK on Tier One Entrepreneur visa and have got ILR today. My wife, who come to UK latter (about 7 months late) on Tier 1 Entrepreneur Dependent visa. Her time lines are as follows:

Initial Entry Clearance visa issued: 24/12/2014
Entered into UK 30/06/2015
Extension valid upto: 26/06/2020

When my wife can apply for ILR at the latest:

28 days before 5 years (Calculated from the date of issuance of visa: i.e. 28/12/2019
OR
28 days before 5 Years ( calculated from her entry into the UK): i.e. 02/06/2020

28 days before 5 years (Calculated from the date of issuance of visa: i.e. 28/12/2019

Is it the SET (O) application I need to file for Tier 1 Ent. Dependent- ILR Application form?

One more question if somebody could answer please...
My daughter who is born in 2000 is 19 years on and full time in Education in University.
I filled up the SET O Child application form, but it did not ask me for her "English Test" and "Life in UK" test details.

Is it correct or I am filing a wrong form?

Please advise...thank you sooooooooo much...

She applies set o on her own as an adult dependent child.

She needs LIUK and English B1 test and proof that she is not living an independent life.
