EU Spouse Visa to France - Sworn statement

Hi,

I am filling out the application for my wife's EU spouse visa to France. They require either ticket or sworn statements by me. How does the sworn statement look like? Do I just print it and sign it? Does it need the notary stamp or my signature is enough? Do you have any examples of that kind of statement?
