Help with Schengen form

Good morning

Filling in my wife's Schengen visa application (first Schengen application) and have a couple of questions on the form.

We're travelling to Zurich, Swz and leaving from Geneva, Swz and we're remaining in Switzerland for the during of our stay (10 days).

I have a British passport from birth, wife has a Pakistani passport and is on a spouse visa.

Question 1)
For #22/23 do I simply state SWITZERLAND for both, or do I need to specifically mention Zurich and Geneva?

22 Member State(s) of destination
23 Member state of first entry


Question 2)
assuming I just state SWITZERLAND?

28 entry permit for final country of destination, where applicable

Question 3)
For #24, what should I state? Ideally we'd like multiple entries so that we don't need to apply every time we want to go to Europe. Any problems if I ask for "multiple entries"?

single entry
two entries
multiple entries

Question 4)
Is that the date of signature and current location? So I put in today's date and "London"?

36 Place and date

Question 5)
For #19,20,31,32,33 can be ignored and not filled in as the application is for the spouse of British passport holder - is that correct?

19* current occupation
20* employer and employer's address and telephone number
31* surname and first name of the inviting person
32* name and address of of inviting company/organisation
33* cost of travelling and living during teh application's stay is covered

* the fields marked * shall not be filled in by family members of EU, EEA or CH citizens


Question 6)
On the documents, I have everything prepared but one thing caught my eye:
We have the last 3 months bank statements, the last one if for end of June and more than 7 days old - is this going to be a problem?

UK Bank statements for the last three months, not older than one week

She can get a visa for the family member of a eu citizen.

Field 21 mark other and write family member eu

Field 22 all schengen countries

Field 23 Switzerland

Field 24 multi entry

Field 25 the number 90

Field 29 your planed travel date

Field 30 travel date plus 179 days

Field 34 your data


Documents for the visa for a family member of a eu citizen:

• One completed visa application
• Valid passport
• 2 biometric passport sized photograph
• Copy of the an EU/EEA or CH citizen citizen’s passport
• Proof of relation to the an EU/EEA or CH citizen
• Signed statement of the an EU/EEA or CH citizen confirming that you are travelling together

Follow up queries on Field 25 and 30 - my wife's passport ends on 12th March 2017 and her BRP ends on 1st March 2017.

In light of this, should I maintain the dates below or do they need to be changed?

Lastly, #28, what does this relate to?

#28 Entry permit for the final country of destination, where applicable

Hi ScoobydooLondon,

Was wondering how you got on here as I was applying pretty soon too.

Where did you go to start applying?
Where did you get the form etc?
Where to register for Visa?

I applied for a Spanish Visa and that was easier to sort out than this.

If anyone else could help too?
