Form AN - Tax Office or Tax Office Reference

3.5 of Form AN asks for a address of tax of office or tax office reference.
How do I find this and what do I put in the form?

Your employer (HR dept) can provide this alternatively the detail will be on your P60.

Does the Tax Office reference number remain the same throughout for an individual?
I can find this on my P45 from October 2012 when I left my previous employer. Unfortunately, all the HR and accounts people are on holiday now at the place I work currently. I have my NCS appointment on 2nd January. So does the Tax Office Number and Tax Office Reference Number remain the same?
Any help will be appreciated. Thanks.

You need to look on your P60. I don't know if the P45 shows the detail that you need. It is a reference number number linked to the employer.
