Health surcharge

Please I have an urgent question, health surcharge for a pbs tier 2 dependent where the main applicant has ILR,

When filling the surcharge form, what date goes for expiry date for the main applicant ?
If I put 2024 the surcharge becomes more than I need to pay, because the extension is for only 2years

Put a date 3 years from now, ie 2022
Char (CR001 not Casa)
