ILR application SET(O) for dependent

Hi,

I have some confusion, could someone please help clarify?

I had Skilled worker visa on which I got my ILR in March. I am now applying for ILR for my husband using the Set (O) form. I have filled the application and taken an appointment date but now very confused while uploading documents. My questions as below:

1. Document requested is 'Declaration signed by Mrs to confirm consent for the Home Office to request verification checks' - Is this referring to Page 2 or 3 of the consent form?
2. After seeing the above, I went back to my application form and realised I have marked both consent questions as No I.e. related to joint acounts, is that going to be an issue?
3. I have not been asked to upload cohabitation evidence, why so? I read a few other forums and then realised that is an important document to upload.
4. Are there any other documents that are not explicitly asked for but we need to upload, similar to point 3?

Part one of the consent form is signed by the applicant and part 2 by the spouse. Ignore part 3.
The document list is not perfect. You need to submit cohabitation evidence: All you need to know about cohabitation evidence

Thanks Zimba for replying so promptly.
What else do I need to provide? What other documents have not been listed?

I am currently only providing:
1. Passport
2. Current BRP
3. All previous BRPs
4. Ecctis letter and degree certificate
5. Consent letter with husband sign in Part 1 and mine in Part 2 as you suggested.

Also, my point no 2 above, would that cause an issue?

The answer on the form is not important. Just sign the consent forms as advised. As far as I know, the only missing item from that list is the cohabitation evidence

One last question, do we need to submit bank statements or payslips for my husband? Again they haven't asked for it so not sure.
