ILR British passport for child

Hi all.
I have ILR since last year, my daugher was born last month and now I need to apply for her British passport. I have received a form from HM passport office but I am confused as to what docs should I send?

Do I need to send my ILR biometric card and my daughter's birth certificate? Or something else is required as well?
