do I need to fill FLR form or tier 2 form.
She will get a 3 year extension as that is what the immigration rules state she will get if you hold ILR or British citizenship. You cannot choose a lesser validity period unfortunately.and she needs extension for one year or 3 years.

If you apply for ILR, she can apply for a PBS Dependent extension once you get ILR. This would be the easier option if her visa has enough validity left (you don't state when it expires).

You will also need to show evidence of 90 days maintenance funds of £630 and pay the IHS of £1200 for a 3 year extension.

Stop listening to 'someone'. A 3 year extension means you have to pay IHS for 3 years, you cannot pay or apply for a lesser duration visa.and some one told me that you will have to pay the health surcharges for 1 year instead of 3 years.
You CANNOT apply for lesser duration, as stated numerous times already. A PBS dependent visa extension is for 3 years once the main PBS migrant has got ILR. This is clearly stated in the Immigration Rules Part 8 and there is nothing you can do to pay for less or for 1 year only.
