She can apply at the same time as you on her own PBS Dependent extension form, separate appointment. However, if your ILR is delayed or refused, her application will also fail.

If you apply for ILR, she can apply for a PBS Dependent extension once you get ILR. This would be the easier option if her visa has enough validity left (you don't state when it expires).

You will also need to show evidence of 90 days maintenance funds of £630 and pay the IHS of £1200 for a 3 year extension.

When was her first PBS Dependent visa ISSUED??

do I need to fill FLR form or tier 2 form.

Do NOT complete FLR(M) form, this will reset her ILR clock to zero and a new 5 year residence (2 x 2.5 year visas) before she can apply for ILR. If you apply for ILR based on 5 years Tier 2 General, she can extend her PBS Dependent visa on the link I have provided.
and she needs extension for one year or 3 years.
She will get a 3 year extension as that is what the immigration rules state she will get if you hold ILR or British citizenship. You cannot choose a lesser validity period unfortunately.

If you don't mind can you send me the application link and some one told me that you will have to pay the health surcharges for 1 year instead of 3 years.

Stop listening to 'someone'. A 3 year extension means you have to pay IHS for 3 years, you cannot pay or apply for a lesser duration visa.

You CANNOT apply for lesser duration, as stated numerous times already. A PBS dependent visa extension is for 3 years once the main PBS migrant has got ILR. This is clearly stated in the Immigration Rules Part 8 and there is nothing you can do to pay for less or for 1 year only.
