PBS Extension for wife/ Main applicant has ILR

Hello,
I recently got my ILR after completing 5 years on Tier 2 General visa. My wife came with me in 2016 but was out of the UK for more than 180 days in 2017/2018 period. So her ILR clock got reset. Her PBS dependent visa expires in July 2022 so we are applying for extension. I have few questions regarding the application.

1) I have selected application under the category skilled worker/partner/extension. is that correct?
2) Under main applicant details I have selected "skilled worker" as it was previously tier 2...?
3) I work in the NHS. There is a new fast track health & care visa under the skilled worker category which has less fee. Can I apply under that category or does it have to be the tier 2/ skilled worker?
4) In the application it did not ask if I was setteled. I read in a few posts that few people got this question. What could be the reason that I was not asked this question?
5) Can I provide multiple bank statements for continous proof of funds?
6) I have included my daughter in the same application as her visa also expires. Would that be ok?
7) I work in NHS so IHS fee exempted for applicants & dependents. But I understand I have to pay for 3 year IHS and then request a refund. Is it true?

What are all her visa start and end dates?? Absence for dependents is only relevant for absence with visas issued on or after 11 January 2018.

Her Visa (BRP) dates are as

1) PBS Dependent Leave to Enter
BRP Issue date : 02/07/2016 Valid Until : 15/07/2019

2) PBS Leave to Remain (Current BRP)
BRP Issue date : 11/06/2019 Valid until : 14/07/2022

Her long absence for child birth in India was from 15th October 2017 to 09th May 2018. Total 206 days.

The period of absence was during validity of the visa issued before 11th Jan 2018. So if I understand correctly this wont be a problem in application of ILR. She meets rest of the criteria for eligibilty. Do I need to write a cover letter during the application or would it be a straight forward application.
Can I also add my daughter to this application even though her stay is less than 5 years.( She is 4 year old and was born outside the UK)

I would greatly appreciate your advice on this.

Any absence on her first visa is irrelevant. Only absence while on the visa issued from 11/6/2019 need to be declare.

You do not need to explain anything. UKVI do know the rules.

The continuity of residence is NOT broken.

Yes you can add the child.

- What category should I select? ILR? Because I dont think there is anything like dependent ILR. If possible can you send a link where I should start on gov.uk?

- Do I need to specify which 5 year period or would UKVI automatically calculate previous 5 years from date of application?
