I am a self-employed dentist and since my arrival to the UK (over 7.5 years ago) I have been working for 2 different dental practices as an associate. I do not trade as a limited company and do not own any of the businesses!

I am now lost with :

3. Evidence that the business is/was actively trading
At least two of the following:
...

-So firstly, they mean "at least two" per year I guess ?
-Any suggestions to what to enclose, as I do not trade directly as a business!
"Actively" trading .... I get every month a payment from the practice I work at, but I will be including that as "Proof of earnings from self-employed"

I wonder if this is for a limited company / business or for a self-employed. I am a provider working for a different companies...I do not do direct advertisement for example...

Also, they ask for "At least two of the following:" rather than "at least two for each relevant year". Which one should I consider ?

You are self employed you said so in your inital post.

Also you should be wary of IR35 (Disguised employment) too, if you are treated like an employee and working practices back that up, you should pay tax as a employer which is normally higher than self-employment.

WRT to the crux of the issue, I expect the case-workier will want to see sales invoices, purchase receipts, accounts, bank statements and records of drawing etc. I would get your accountant to provide these.
