Immigration Health Surcharge for Tier 1 dependent newborn

Hi there,
Wonder if anybody has faced the same issue. I am applying for my daughters Tier 1 dependent Visa; she was born in the UK a few weeks ago and has received Indian passport. I have come across a situation where my visa category is not listed in the dropdown. Here are the details I put in

Lead applicant : my daughter details
Visa Route: PBS Dependent (Tier 1,2 and 5)
Visa Type : Tier 1 General

Main Applicant: my details
Visa Route: Tier 1
Visa Type: *CANT FIND Tier 1 GENERAL HERE*

So my questions are
1. What is lead and main applicant? Am I filling it the other way round?
2. What did you guys do if you had similar issue?

If you are only applying for a dependent, then they apply alone in regards to the surcharge. Don't put main applicant information.
