Place and date of spouse declaration

Hi all

We had our spousal declaration returned due to missing the space to where it asks the place, and day and month at the bottom when we had the declaration witnessed by a notary public.

The person returning the form highlighted the date we did the declaration and highlighted the blank spaces.

When we go back to the notary today, do we redo it with today's date? I would have thought so, but the highlight of October's date is confusing to me.
