Medical Treatment in the UK

Hi,

I am applying for my wife's dependant visa (Tier2) extension. On the application one of the questions is about medical treatment history in the UK. It asks for details such as if we visited A&E or at a doctor's surgery or clinic or hospital appointment and when these treatments started and ended. Honestly I have no record of all these visits and when we went etc. How would I fill this?

Any help is much appreciated!
