PBS tier 1 dependent

Hi,
I am on a dependent workpermit and my husband switching to Tier 1 general. I am confused and not sure if i have the right application form for dependent tier 1.
the first page on the PBS dependent form says:-"Application for an extension of stay as a dependant of a person who has leave to remain under the points based system". But i am not on tier dependent.
Could any of you point me to download the the correct PBS dependent application form please.

Workpermit is replaced by Tier2(G) so in future documents WP will be refered as Tier2
