Question about MN1

I got ILR a few weeks back and want apply for registration of 2.5 years old child as British citizen using FORM MN1.

I am not able to find professional refree. GP and Health Visitor refused. No body else is engaged with child in professional capacity.

Q1. Given the circumstances will following refrees be acceptable:
1. A friend who is doctor
2. A colleague who is IT professional

Q2. According to my understanding I have to pay £673 fee. Is it right or I am missing something?

Also see Home Office is more lenient with mn1 applications, specially if it concerns very young children..
