As per my understanding, if you book an appointment at premium or enhanced centre then the scanning cost would be complimentary. Otherwise you might have to pay. Refer to link as above.

Thank you CR001 for being helpful. If the initial visa was granted in January 2015, then no you cant. You need 2 FT “JOBS” for minimum 12 months each to qualify.
