Once you are refused you then fall into new rules. This only applies if new rules have come into affect after the first application. In your case you applied again after 9th July 2012 and therefor will be subject to the new rules.

Yes, get proof from both employers that you earn x per month. Possibly deposit cash into bank so that it reflects on your bank statements as you will need this again later on when you apply for FLR or ILR.

Congratulations, 3 months is not that long. If you go the fiancé route to enter the UK and marry whilst there, then long delays occur to get FLR (m). You cannot work either until you get your biometric resident permit.
