Tier 4 visa wrongly refused for non-submission of TB test certificate

The tier 4 visa was refused in New Delhi on the grounds that TB Test certificate was not provided in VFS on 14.06.19 (date of submission of file) but actually, we submitted the medical certificate . The certificate is dated 15.05.19 . I also have a copy of the certificate. Probably, VFS did not scan the test certificate
Can someone answer these 3 questions :

(1) Incase I want to apply for a new visa before the decision for my administrative review is given, do I need to send an application to administrative.reviews@homeoffice.gov.uk for withdrawl or on applying for new VISA , automatically previous request for review is null & void ?

(2) After successful review , how many days to get the passport with VISA ? Is there any way to expediate this as Tier 4 visa is timebound ?

(3) Course in UK starts on 02.09.19 . Is there a deadline after which university cannot or will not issue new CAS ?

I have 3 questions if someone can help :

(1) If before decision is awarded in Admin review , I decide to withdraw the review & apply for new VISA - do I need to send email to some ID for cancellation of review or is it automatically withdrawn ?

(2) if time is less & course is about to begin , & even though I get a favourable decision from Admin review but I apply for new VISA on priority basis - in that case , will the VISA refusal be struck off from my record (as I got favourable decision) ? Remember , here a new CAS will be used for VISA purpose.

(3) In case admin review is in my favour & I get UK Student VISA in my passport - in future , when filling any new VISA application etc. if asked " HAVE YOU EVER BEEN REFUSED A VISA- do I write YES or No to this question ?
