Need help for T-1G depnd app form

Can any one help me which application form do I need for extension of my wife;s tier-1 general dependent visa as she has not completed her 5 years and I am going for ILR next week in Cardiff counter.

Also is Cardiff good center as compare to Shehfied or Croydon? Please help out as i am still waiting amended tax return for year 2010/11 from HMRC for last 3 month or so??/
