how to appeal a Family visitor Visa Refusal

Hello every one in the Forum,
I applied for my sister in law to come to visit us for 2 weeks and her application has been refused because they are not satisfied that She meets the requirements of paragraphs V4.2 - V4.10 of appendix V.

When i applied for he visa we sent the following:
1- proof that She has 2 dependant Children will not travel with her
2-Proof of her Employment.
3-Bank Statements with £4000 Balance
4- invitation letter from me and I declared that I will be responsible for all the costs and accommodation in UK.
5-proof of my accommodation and a letter from the Landlord.
6-My bank Statements with good Balance.
7-proof of my employment.

please any one can help me in how to challenge this decision and apply for appeal.

What was the exact refusal letter wording (taking out any personal information).

It is the applicants responsibility to provide as much documentation as possible to prove their ties to home country to convince the ECO that they are genuine visitors.

Where does the £4k come from?

What about her payslips?

Did she have a letter from employer granting her annual holiday etc?
