PBS dependent Extension

Hello,
I'm planning to apply for PBS extension for my wife. I have few queries. It will be of great help if anyone could answer please?

Does health surcharge fees is applicable for PBS dependent visa extension? My wife's current situation is she is currently on a PBS dependent Visa from Feb 2014 and it is valid till Apr 2016. I'm planning to apply for extension which will be 3 years( I got my ILR in between). Please let me know whether health surcharge is applicable?

Also can you please help me with differentiating between below two options which is part of the application?
1. Tier 2 General, ICT Long Term Staff – over 3 years leave to remain
2. Tier 2 General, ICT Long Term Staff – upto 3 years

What category of PBS dependent visa does she hold, i.e. what was your visa before ILR?
Char (CR001 not Casa)
