Bringing Child Back from India to UK - Tier 2 general dependent visa

My child is along with my parents back in india. Due to covid, the plan to bring child along with grandparents has been cancelled. I am planning to go and bring her on my own with my wife staying in UK.
My wife works with nhs and am in general visa.
So with the background I have a few questions which it would be great if you could help on
1. What is process of utilizing the care visa so that ihs surcharge is not being charged
2. Do we have any format of consent letter
3. Which are the supporting documents that would be helpful for the applications
4. In the application, it asks to mention who will be accompanying the child - could we provide an option of either me or wife
5. How much long than a month would we get from time of getting visa to be back to uk
6. Has the visa centres opened to start processing the visa in kerala, india. Any advice on doing appointments with vfs centres in kerala considering the current covid

If you hold a tier 2 general visa, you have to apply for a tier 2 general dependent visa for your child and pay the IHS.

What visa does your spouse hold?? Presumably a tier 2 dependent visa.
