skilled visa update queries

Hi ,
I am a bit confused reading the ruled . My friend is on a skilled visa for 3 years .
Could I ask what exactly updating a skilled Visa means when he changes to a different company but stays in the same job

Does he have to pay the whole 3 years again ? and also IHS fee for the 3 years ?

Or is it just a simple update process and nothing to pay?

If he changes employer, he has to apply for a new skilled worker visa and pay the visa fee and IHS fees again.
