Form AN Application queries - Please help

Dear friends
I am planning to apply for naturalisation soon, I have some queries with filling out Application form AN, please could you review and give me some guidance on how I can answer them?

Knowledge of language/life in the UK
I have passed Life in the UK test last year. And also I have satisfied English language requirements during my Tier 1 General renewals and ILR applications. I have a Masters degree from USA and is recognised in UKBA universities list in points based calculator. I am planning to respond below for the below question, please let me know if I am right.
1.22 How do you intend to satisfy the requirement to have sufficient knowledge of language and life in the UK? See pages 12 to 16 in the accompanying Booklet AN
I have passed the Life in the UK test and attach confirmation: YES
The test number is: XXXXX
And either
Or
I met the knowledge of language and life requirement to qualify for settlement on or after
28th October 2013; YES
Shall I answer to both of the above points, or shall I just tick the second one which includes both language and life? I received my ILR in March 2016.
Details of your employment
I am a contractor working through my own limited company full time.
1.46: What option should I choose Self employed or a director?
1.49: Address of tax office or tax office reference: What does this mean please
Section 6: Biometric enrolment
I am planning to apply via NCS, and will fill the details in questions from 6.1 to 6.12 with all my current BRP card (ILR) details and confirm that I am not enclosing it, and will cite reasons for that as I am submitting via NCS.
I could not understand the next set of questions, which are below, Do I need to answer these? Could you please explain what these questions are?
6.13 Date your fingerprints were taken: D D M M Y Y Y Y
6.14 Give details where your fingerprints were taken, including the town or city and country:
6.15 Give details of the British diplomatic post(s) involved if the application(s) was or were
made abroad:
6.16 Do you have a medical or physical condition which may require special arrangements for
your biometric features to be recorded?
Yes please provide us with a letter from a doctor registered with the General Medical
Council (GMC) giving details of the condition and/or special needs and explaining any
arrangements that may be necessary.

6.17 Declaration
As required by British Nationality (General) (Amendment) (2) Regulations 2014, I confirm that I
wish to register my biometric information.
Lastly, in Supporting documents section
•Evidence of tax for self-employed applicants only. The most recent HM Revenue & Customs Self Assessment Statement of Account
I currently have myself and my wife being shareholders of limited company, do I need to provide Self assessment statements for both of us, or mine should be fine. I added her only last year to my limited company and I have not filed one for her yet. I think this is due later this year.
Please could you review and respond to my queries please.
