Your citizenship will be granted in the name of your EU passport.
There is NO requirement to submit a birth certificate

Hi
I m an EU holding both Permanent Residence since Nov 2018 and Settle Status Sept 2019 . If I m applying for British Citizenship can I apply on basis of Permanent Residence or my Permanent Residence is no longer valid after getting Settle status ?
Arun

Yes, you can use your DCPR to apply as soon as you've had PR status for at least 12 months.
That is, if you acquired PR status on 1 Nov 2018 you can apply no earlier than 1 Nov 2019. Note that the date on the DCPR is not the date you acquired PR status, but the date the DCPR was issued which may be later than the date you acquired PR. Your DCPR approval letter should tell you the date you acquired PR status and hence the date (a year later) that you can apply for citizenship.
