Gap between previus visa end date and new BRP issue date

I'm planning to apply for ILR (LR) in August 2018. While checking my previous visas I found that in one of my visas there is a gap between previous Tier-4 dependant visa end date (05-05-2013) and Tier-2 general BRP issue date (28-05-2013). But I definitely applied before my visa end date (05-05-2013) being in UK. Would it be considered as discontinuation of my LR?

Please let me know if I understood well....the continuation is maintained since the application was pending with HO and it was granted although BRP issued on 28-05-2013.

Set(LR) form fill up question

I’m filling up my SET(LR) form. There is a question such as:
“D18. Please provide the name, nationality and date of birth of any children who reside with you in the UK. Please provide evidence of their residence with you.”

I’ve two children aged 11 and 2 who both are born in UK and reside with me. What documents should I provide as evidence of their residence with me? I’ve some letters from GP, hospital or from school/ in different occasions on their name.
