help with filling in eea3 form

Can anyone help for filling in eea3 form?
We are a family of 5: me, my wife and three children, 6 months, 6 years and 3 years old. The children are all born in the UK. We are Romanians living in the UK since 2004 on a work permit basis. After 2007 me and my wife applied for the blue card- registration certificate and obtain it.
Now we would like to apply for British citizenship but, as I understand, we need to apply for permanent residence first using the eea3 form.
We are not sure which sections( section 1, 2 and 3) of the form to fill with which details and whether to include any children in it.
