FLR(M) Visa Extention

Hi Guys - Need some advise.
  • I currently hold an ILR and I am applying to extend my wife's FLR(M) visa which expires on 29-12-2020. She has already given her IELTS Academic exam (04/2018) when FLR(M) was initially applied (06/2018) and has CEFR Level C1. Is she required to take another IELTS exam for the extention given IELTS is only valid for 2 years. ?
  • She is working as a Locum doctor in NHS. So, should she pay the NHS surcharge fee as part of the application now?. She had to pay for the initial FLR(M) application as she wasn't exempt but now not sure she is excempt as per Boris' announcement

No if that test was previously accepted and also have UKVI reference number
