Urgent!!!! Need Help on Tier2 General COS and Visa

Hi Manci and Experts,

I am switching from Tier2 ICT to Tier2 General within UK.
I have couple of queries .
1. In my COS there is a question like
[b]Does the migrant need to leave and re-enter the UK during the period of approval?[/b] Y
I am not sure if 'Y' is the right answer ..Is this something required if you want to travel outside UK with your T2G visa, please advise?

2. I have booked Tier2 General appointment online sometime during March for the appointment in Apr 3rd week and made the payment online at that time.
However there is a change in fees for applicants applying after 06'Apr.
My query is , Is it fine with the old Payment Receipt as there is an increase in fee but i paid online with the reduced fee (before 06'Apr)
Also what Visa form should i use while appearing for PEO appointment. Is it Latest Tier2 appl form (04/2014) or the old one?
