put the 2009 date in 1.3, attach your PR Confirmation booklet. You can skip section 2.4-2.6 (not required as you hold PR Confirmation for more than 12 months).
Check the FAQ before posting!
Citizenship (adults, children, passport)
EEA (EEA FP, RC, PR, Surinder Singh)

I've done the whole research on the subject in the forum yesterday and came across your advice in similar case.
I was going to do exactly the same and send application myself.
